                                Community Surveillance Coordinator
JOB SUMMARY:
Under the authority and general direction of the Sandy Lake First Nation Chief and Council and
partnership with the University of Western Ontario and through the instructions of the Sandy Lake First Nation Health Authority the Community Surveillance Coordinator will be responsible for developing and maintaining a complete community diabetes registry ( a list of adults 18 years and older only with type 2 diabetes.

 

 
 

 


JOB DESCRIPTION:
The Community Surveillance Coordinator is the person responsible for developing and maintaining a complete community diabetes registry (i.e. a list of adults 18 years and older only with type 2 diabetes). The Community Surveillance Coordinator is responsible for scheduling appointments with the community healthcare facility and reviewing medical charts on the premises. Successful candidates will be responsible for carefully collecting confidential medical data and imputing the information into a web-based surveillance system. Essential to this position will be building collaborative relationships within the community and with the healthcare facility. Training will be provided by The University of Western Ontario. Candidates must be available to travel for training and committee meetings outside the community. This is a 12-month full-time position, working 35 hours per week and may include evenings and weekends. Flexibility in scheduling is essential.
KEY RESPONSIBILITIES:
  1. Act as the Community Surveillance Coordinator raising awareness of the project and building collaborative relationships within the community (community leadership, healthcare facility, community wide)
  2. Attend a training session held by The University of Western Ontario. Training may be held within the community or outside the community and may require a few days of travel
  3. Develop and confirm completeness of a community diabetes registry (adults aged 18 years and older only)
  4. Maintain and update the community diabetes registry as needed
  5. Enter confidential diabetes information from patient medical charts into a web-based surveillance application
  6. Ensure project timelines are met and all information collected is accurate and complete. This will involve checking completed work for errors or duplicate information before submitting data
  7. Participate in community dissemination where results of the project are presented back to the community
  8. Sit on Community Advisory Board and National Steering Committee and provide project updates, discuss issues/challenges and guide project activities to ensure community relevance
  9. Other assignments as requested by The University of Western Ontario or Project Manager
KNOWLEDGE AND SKILLS:
  • High school diploma completion as a minimum. Preferred bachelors or masters degree in Health Sciences or other health-related discipline (Social Sciences)
  • Reading medical information
  • Working independently and collaboratively with others
  • Takes initiative to problem solving with good decision making skills
  • Job task planning requiring excellent organization skills and ability to document procedures
  • Computer use including Windows, electronic mail and Internet browsers is a must
  • Attention to detail, accuracy, and the ability to pick up new skills quickly are essential, as are friendliness, punctuality and an awareness of privacy and confidentiality in research
  • Interest in diabetes and/or research is a bonus, as is experience working with health care providers in the community
  • Preference will be given to experienced chart auditors
  • Experience in a research field and diabetes knowledge
